Hospital to Home Consultation
Discharging home from the hospital after a surgical procedure, fall, or new diagnoses can be a very scary feeling. We want to assist with welcoming you or your family member home the right way by assisting with creating an environment that is conducive to meeting your current medical needs. This virtual service is design to support you and/or caregivers in identifying accessibility barriers within the home environment, navigating the healthcare system with confidence, improving health literacy and education on proper usage of durable medical equipment prior to discharging from the hospital.
Aging In Place Consultation
Living in your home environment as you age takes some careful planning and preparation. We want to help you be proactive, not reactive when it comes to setting your home environment up for daily success, maintaining your independence, and improving your safety. Our trained home modification consultant will tailor each consultation to meet your specific needs.
Home Safety Assessment (Non-Medical)
A non-medical home safety assessment is a basic evaluation of your physical home environment to identify and eliminate barriers to independence and potential fall hazards. A home safety assessment is ideal if you are preparing to have an elective surgery, discharging home from the hospital and/or recent hospitalization, caring for an aging parent, or had multiple falls. Our skilled medical professional will tailor each home safety assessment to fit your individual needs or the needs of your family member.
Comprehensive Home Modification Assessment
A home modification assessment is a comprehensive evaluation of your physical home environment, client factors and your ability to function within your home environment safely. This assessment is designed to take a deeper look into your medical history, daily routine performance and identify environmental factors within your home that impede or hinder your performance with your selfcare tasks. Our trained medical professional will recommend low to high costs home modifications that are specific to your individualized needs, goals, and functional status. This assessment is ideal for individuals undergoing major home renovations (shower remodels), individuals with catastrophic injuries and workman compensation related injuries. We want to partner with you to assist in making your home environment work for you not against you.
Our Home Assessment Process
Step 1: Schedule your Complimentary Discovery Call (15 min)
During this important step we gather basic information from prospective clients to understand your unique needs and goals to identify if our services will be a good fit for you.
Step 2: Schedule your Home Assessment (We offer Virtual or In-person Assessments)
We provide a thorough assessment of the home environment to identify areas within your living space that may present barriers to independence, safety, and accessibility. This includes but is not limited to doorway entrances, bathrooms, and bedrooms.
Step 3: Customize Plan Development
We collaborate with you to devise a personalized plan with recommendations that offer solutions to your problems areas such as: grab bars installations, widening doorways, ramp installations, bathroom renovations and adaptive equipment recommendations.
Step 4: Contractor Selection/Coordination of Services
We help you find qualified handyman, contractors, and vendors in your area to do the work for you. We do the hard work of getting quotes and coordinating these services on your behalf.

Step 5: Education and Training
We offer education and training sessions as needed to ensure your understanding and are comfortable with the safe usage of any adaptive equipment and any new home modifications installations.
Step 6: Follow-up and Support
After the completion of the home modification and/or renovations, we conduct a final check-in to ensure your satisfaction and address any concerns you may have.
